Our sincere apologies for the name error - when the request was transferred from the Department of the Prime Minister and Cabinet to Police, we were advised that was your name (see attached letter).
Police do not hold prosecution data, this is held by Ministry of Justice as they oversee the judicial system - however Ministry of Justice declined the transfer request from the Department of the Prime Minister and Cabinet.
Therefore in order to assist with your request, Police advised Department of the Prime Minister and Cabinet that they can provide Covid-19 Alert Level 4 Breach data for August-September 2021 (the number of infringements notices issued to people who were non-compliant with the Public Health Response Alert Level Requirements Order No 9 2021).
